Each individual will have the following provided for them:

• Privacy / Respect / Diginity

• 3 Meals daily, plus Snacks / assistance if needed

• Laundry / personal and bedding

• Activities / excersise / as directed by Family or Dr.

• Daily Cleaning of Rooms

• Bath room assistance if needed

• Bathing assistance if needed

• Reminders for meds / assistance if needed

• Generial overall observations and regular reports to family

We will work with each persons family to set up schedules for med refills, activites, and Dr. appointments.  Please see level chart for further details.

                                                        LEVELS OF CARE

LEVEL 1        $ 850.00
Person can not live independently and wants housing supplied with 24 hour supervision. The person must be able to provide their own transportation, purchase and prepare their own food as well as shop for themselves. This individual must be able to bath and groom themselves. Access to laundry, handicap bathroom, kitchen, and common living area provided. Common living area will be available for visitors and family. Reminders to take medication will be set up and directed by family and followed up by caregiver. Overnight stay available for family. Our family will include this individual in daily activities, and provide needed companionship as desired..


LEVEL 2        $1250.00
Person can not live independently and wants housing supplied with 24 hour supervision. Care giver will supply food and laundry supplies, assist with bathing and grooming, assist with laundry, assist with transportation ( sun van, public transportation…etc.) assist with food prep 3xp/day. Common living area will be available for visitors and family. Reminders to take medication will be set up and directed by family and followed up by caregiver. Overnight stay available for family. Our family will include this individual in daily activities, and provide needed companionship as desired.


LEVEL 3         $ 2500.00
Person can not live independently and wants housing supplied with 24 hour supervision. Person needs minimal assistance to be ambulatory / walker / wheel chair. Care giver will help with toileting, bathing grooming and feeding Meals will be prepared 3X daily along with snacks as requested. Transportation provided to and from Dr. Appointments. Common living area will be available for visitors and family. Medication will be set up and directed by family and followed up by caregiver. Overnight stay available for family. Our family will include this individual in daily activities, and provide needed companionship as desired.


LEVEL 4 ( up to and including Hospice) $ 3500.00
Person requires 24 hour supervision and care. All meals and feeding provided. Care giver is experienced in total hospice care for 17 + years. Common living area will be available for visitors and family. Medication will be set up and directed by family / hospice and followed up by caregiver. Overnight stay available for family. Our family will include this individual in daily activities, and provide needed companionship as desired.
